How to Register to be an Organ, Eye and Tissue Donor at the
Department of Motor Vehicles
If you’re planning to renew your New York State driver license, apply for a new license, or obtain a non-driver photo ID card, you have the opportunity to register as an organ, eye and tissue donor.
When you fill out your driver license or non-driver ID card application (form MV-44), there will be an area in which you can sign to enroll in the New York State Department of Health’s Donate Life Registry:
Just check YES! Then sign and date this section.
Your signature indicates you are legally authorizing organ, eye and tissue donation upon your passing. It authorizes the DMV to give your information to the New York State Department of Health for
enrollment in the Donate Life Registry.
You will receive a letter of confirmation from the Department of Health, which will also provide you with an opportunity to limit your donation.
Do You Have a Heart?
If you enroll through the DMV, as part of a license, permit or non-driver ID transaction, a symbol of a heart and the words “ORGAN DONOR” will be printed on the front of your document at no extra cost.
So when someone asks you: “Do you have a heart?” you can proudly answer: “Yes!”
